EMERGENCY MINI-MEDICAL RECORD TEMPLATE
I designed the Emergency Mini-Medical Record Template to meet an unfilled need within the pediatric special care community– the need to have just the right amount of information on hand when you need to bring your child to a hospital that isn’t your medical home: more than the bare minimum, but less than a binder of information that may be overlooked by busy medical staff.
This downloadable template is meant to be filled out on a computer, printed out, and brought along on trips or kept on the front of the medical home binder… in case the unexpected happens.
